At Rehabilitation Hospital, we want to make sure you don't encounter any unwanted surprises as a patient. That's why we've included information on everything you need to know about your stay in this section. Rehabilitation Hospital accepts patients 14 years of age and older.
Visitor Information
Official visiting hours are 8 a.m. to 8 p.m. daily. You should limit your guests to two at a time. In order to ensure your well-being, the nurses and therapy team have discretionary authority regarding the number of visitors and the length of stay. However, we know that sometimes the best medicine for anyone is having loved ones nearby. That's why we sometimes make exceptions to the rule. If you have family members who are unable to come during the designated visiting time, please notify your nurse so that special arrangements can be made. Family members or friends may also eat with you in the dining area by paying a nominal fee.
Adults and children with a communicable disease must, of course, refrain from visiting.
Rehabilitation Hospital is a smoke- and tobacco-free hospital. Smoking, including the use of e-cigarettes, vaping devices and other similar products, along with the use of any other tobacco products, is prohibited in the hospital and on the campus grounds. Alcoholic beverages are also prohibited.
Advance Directives
Each individual has the right to direct his or her own care. Current information about Advanced Directives and Indiana state forms can be found on the Indiana State Department of Health website.
Forms included, but not limited to, include:
- Health care representative appointment
- Life prolonging procedures declaration
- Living will declaration (English and Spanish versions)
- Out-of-hospital do not resuscitate declaration and order
