Rehabilitation is the process of helping patients recover and regain their quality of life. We use the Functional Independent Measure (FIM) tool to provide an accurate assessment of progress. The measured outcomes are used to improve patient care, treatment plans and staff education.
From admission to discharge, and even after discharge, we know how much progress each patient has made in key rehabilitation areas. This allows us to not only be confident in their abilities, but also to identify areas that need to be addressed further to ensure the maximum level of functional independence.
The Uniform Data System for Medical Rehabilitation (UDSMR) tracks and trends information regarding outcomes on our behalf to actively benchmark our performance against national averages. Highlights include:
- 73% of Rehabilitation Hospital patients return to the community after completing their rehabilitation
- Average length of stay at Rehabilitation Hospital in 2016 was 11 days compared to 13 days for the national average
With our measured outcomes initiatives, we are able to provide clear, industry-benchmarked information. The Rehabilitation Hospital of Fort Wayne constantly evaluates data to identify areas for improvement.
