Care Planning
Our patients require interdisciplinary care for effective treatment. In order to maximize our efforts, we treat each patient individually. Physicians, nurses, therapists, clinicians, case managers, patients and their families work together daily to develop and provide coordinated, effective care.
Weekly team conferences are held with multiple team members in attendance including the admitting and attending physician. Areas such as medical needs, therapy progress, discharge planning, patient's participation and motivation are discussed as a team. The case management staff updates patients and families with progress, treatment plans, estimated length of stay and discharge options.
Patient Involvement
We make every effort to include patient and family input in these meetings. The patient's goals are an essential component of our planning.
We also schedule family conferences where the team discusses patient and family concerns, provides updates and discusses discharge planning. We work with the patient and family to set realistic, attainable goals. Case managers attend these meetings as the patient's advocate (or as a liaison if the patient is unable to participate). Case managers and all staff members are available to answer questions.
Medical Directors
The patient's care is coordinated by the admitting/attending physician and is followed by one of our medical directors. The physician evaluates, approves and assesses the care plan of each patient. Consultation with medical specialists are afforded as part of our comprehensive physician coverage. We welcome family physician involvement.
