  • The Joint Commission -- Rehabilitation Hospital of Fort Wayne is committed to patient safety. Please call (260) 435-6103 or email us to share any compliments or concerns. Or, you may call (800) 994-6610 to share any comments regarding patient safety with The Joint Commission.

Medication List

You should take an active role in documenting and maintaining an accurate, up-to-date listing of your allergy and medication history. This list should include all prescription and over-the-counter medications as well as herbals, nutritional supplements and vitamins that you are currently taking. You should note dosage, strength and time taken. The healthcare team needs to know this information to provide the best possible care. This information should be kept with you at all times and updated whenever a change occurs.

Restraint-Free Policy

The Rehabilitation Hospital is a restraint-free hospital. We believe that in order to support our philosophy of rehabilitation it is essential to eliminate the use of chemical and physical restraints. We use alternate strategies such as bed and wheelchair alarms, low beds, locating patients in rooms within close proximity to the nurses' station, one-on-one supervision and diversional activities as alternatives.

Tobacco-Free Campus

The Rehabilitation Hospital of Fort Wayne as well as all Lutheran Health Network hospitals in
Allen County are tobacco free. The use of any tobacco products (cigarettes, cigars, chew, etc.) are prohibited on the campus for employees, volunteers, patients, medical staff, visitors and vendors.

Secondhand smoke has been proven to be harmful. In the healthcare environment, it is especially important that those who may have compromised immune systems or other health conditions, that leave them more susceptible to the effects of secondhand smoke, not be exposed to it while on the hospital campus. The tobacco-free policy was not created to infringe upon the right of personal choice, but instead, it was created to promote and protect the health of those for whom we provide care.

Alcoholic beverages are also prohibited at Rehab Hospital.
