Rehab Hospital is located in Fort Wayne, Indiana, near the intersection of I-69 and US 24 on the Lutheran Hospital campus.

From Interstate 69

From I-69, take exit onto US 24 east (as if going toward downtown Fort Wayne).
Turn left at the first traffic light (less than one-half mile from I-69) at the entrance to the Lutheran Medical Park.
Once in the medical park, follow the signs to the north campus.
Turn right (north) on the first cross-street.
Rehabilitation Hospital is the one-story building on the right side of the road.

From the east/north on US 24 (Jefferson Blvd in Fort Wayne).

Go through downtown and drive south (southwest) through several retail areas. Engle Road is the last marked crossroad/traffic light before the Lutheran Medical Park entrance. The entrance is approximately one-half mile south of Engle Road. There is a traffic light at the Lutheran Medical Park entrance.
Turn right (west) into the Lutheran Medical Park.
Once in the medical park, follow the signs to the north campus.
Turn right (north) on the first cross-street.
Rehabilitation Hospital is the one-story building on the right side of the road.

From Fort Wayne International Airport

Start out going northwest on W. Ferguson Road toward 2nd Street.
W. Ferguson Road becomes Indianapolis Road.
Turn left onto Airport Expressway / Dalman Road. Continue to follow Airport Expressway.
Airport Expressway becomes Lower Huntington Road.
Merge onto I-69 N / US 24 W / US 33 S.
Take the US 24 W exit (exit 302) toward Huntington/Fort Wayne.
Take the ramp toward Fort Wayne.
Merge onto W. Jefferson Blvd.
Turn left at the first traffic light (less than one-half mile from I-69) at the entrance to the Lutheran Medical Park.
Once in the medical park, follow the signs to the north campus.
Turn right (north) on the first cross-street.
